Not all haemorrhoid surgery is the same. The MOH Singapore and the colorectal surgery community use a four-grade classification system to determine the appropriate level of intervention. Your grade directly influences which surgical procedure is recommended and therefore what you will pay.

Grade I and Grade II: Office-Based Procedures

Grade I haemorrhoids bleed but do not prolapse. Grade II prolapse during a bowel movement but retract spontaneously. Both are typically managed with minimally invasive outpatient procedures that do not require general anaesthesia or an operating theatre.

Grade III: Surgical Intervention

Grade III haemorrhoids prolapse during straining and require manual reduction. Conservative management and office procedures are usually insufficient at this stage. Surgical treatment under general anaesthesia becomes the standard of care. The three main surgical approaches — Conventional Hemorrhoidectomy, Stapled Hemorrhoidopexy (PPH) and Laser Hemorrhoidoplasty (LHP) — are all appropriate for Grade III disease. The choice depends on anatomy, patient preference and case complexity.

Grade IV: Surgery Is the Definitive Treatment

Grade IV haemorrhoids are permanently prolapsed and cannot be manually reduced. They carry a significantly higher risk of strangulation, thrombosis and ulceration if left untreated. Surgery is not elective at this stage — it is the recommended and necessary treatment. The case complexity associated with Grade IV disease may increase surgeon fees, operating theatre time and the likelihood of an inpatient rather than day surgery admission.

Laser Hemorrhoidoplasty (LHP)
